Dear "Your Doctor's Name"
A patient in your practice is interested in receiving office-based treatment for opioid dependence and would like to encourage you to become a certified SUBOXONE® prescriber.
Opioid dependence is a chronic medical condition. In 2003, an estimated 2.4 million people in the United States were dependent on or abusing opioid analgesics and/or heroin. The stigma attached to opioid dependence can be so overwhelming that thousands of patients every year choose to continue using rather than risk possible exposure by receiving treatment.
Because SUBOXONE is approved for office-based treatment of opioid dependence, it allows for privacy and confidentiality in the treatment of this disease. SUBOXONE suppresses withdrawal symptoms, decreases cravings, and improves treatment retention.
According to the DATA 2000, if you meet any of the following criteria, you are considered prequalified to prescribe SUBOXONE for opioid dependence:Physicians can become qualified to prescribe SUBOXONE by participating in an 8-hour, DATA 2000—approved training course. Information about upcoming training sessions and registration is available at www.DocOptIn.com and www.bupenorphine.samhsa.gov, or by calling 1-888-DOC-OPTIN (1-888-362-6784).
All physicians must submit a Notification of Intent to treat form (SMA 167) to the Substance Abuse and Mental Health Services Administration (SAMHSA) for processing before they are legally recognized as DATA 2000 certified. Additional information about training and other aspects of becoming certified is found at Suboxone.com under Frequently Asked Questions.
Thank you for considering offering opioid-dependence treatment and SUBOXONE to your opioid-dependent patients. Today, someone in your practice looks forward to beginning office-based treatment with your professional assistance.
Sincerely,
Rolley E. Johnson, Pharm.D.
